

Keep Memory Alive and the social services staff at Cleveland Clinic Lou Ruvo Center for Brain Health provide individualized assistance to caregivers and families of patients. Guidance is offered for planning for care of a loved one over the course of a neurocognitive illness as well as supporting the wellbeing of the caregiver. Key components of this service include:
Assessment – This initial step is an opportunity for the caregiver to share information about the caregiving experience, recognizing the uniqueness of each individual and their family situation.
Care Planning – Using the information gathered during the assessment, the social worker and caregiver work together to develop a plan of care to resolve immediate concerns and establish a process to address challenges that may occur as the disease progresses. A plan of care includes the specific steps that will be taken to achieve the desired outcomes.
Counseling and Support – Focusing on the needs of both the caregivers and the patient, we offer education, support and referrals to community resources. This may include assistance with specific issues related to providing hands-on care, problem-solving strategies to address changes in behaviors due to the illness and approaches for enhancing the coping skills of the caregiver and members of their support network. Therapeutic counseling is also available to assist caregivers in maintaining their emotional wellbeing while minimizing negative effects associated with caregiving such as depression, anxiety, adjustment to change and grief.
