Medical Appointments

Your doctor has referred you to a specialist at Cleveland Clinic Lou Ruvo Center for Brain Health. Now what? Following is a brief overview of what to do before and during your initial visit. Make an appointment

  • Call 702.483.6000 between 8 a.m. and 5 p.m. Monday through Friday
  • Have your medical insurance cards handy

The day of your appointment

You may want to bring a family member or friend with you. You should also bring:

  • A government-issued photo ID
  • Your medical insurance card(s), including prescription drug card
  • Copies of any medical-legal paperwork (power of attorney, guardianship, advanced directive)
  • Your primary care provider’s name, phone number, and fax number
  • Your pharmacy’s name and address or phone number
  • Names and phone numbers of people you authorize to receive your medical information
  • A list of the most important questions you want to ask the provider
  • A list of your current medications and supplements: names, doses, and schedule
  • Results of recent lab work or other tests
  • Product cards for any implants you have
  • CDs of all previous brain or spinal cord imaging (new imaging will likely be conducted at the Lou Ruvo Center for Brain Health)

While this information is helpful to us, please do not worry if you’re missing something or you forget it. The Lou Ruvo Center for Brain Health will work with you to get the information we need.

Check-in for your appointment on the first floor

  • Arrive a half-hour before your scheduled appointment time
  • After check-in, you’ll be escorted upstairs to begin your appointment

Visit with your provider

  • Let your provider know you have a list of questions
  • Ask about participating in clinical trials or research studies
  • You will receive an after-visit summary that explains the plan of care

Check out

  • A Patient Service Representative will work with you to arrange for upcoming visits and testing

Sign up for MyChart, Cleveland Clinic’s electronic medical record system, if you didn’t do so at check-in. MyChart gives you the opportunity to address questions that you didn’t get to during the appointment or issues that arise between visits.
